  • Authorized Caregivers (if you are authorized by a patient or the provider): name, relationship to patient, email, phone

1.2 Protected Health Information (PHI)

Because we are a healthcare platform, we collect PHI consistent with HIPAA definitions: information about a patient’s health condition, medical treatment, payment for healthcare, or that identifies the patient.

This may include:

  • Past, present, or future physical or mental health states
  • Healthcare provision details
  • Billing or payment information for healthcare services
  • Any information that reasonably could identify a patient (e.g., name, date of birth, contact details)

1.3 Non-Personal / Non-Identifiable Information

We also collect Non-Personal Information, which does not identify you personally. This includes:

  • As de-identified or aggregated data that does not identify individuals
  • Mobile information will not be shared with third parties/affiliates for marketing/promotional purposes. All the categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties

We treat PHI in accordance with HIPAA, HITECH, and related regulations.

  • Business Associate Agreements are used where required
  • PHI is limited to treatment, payment, and healthcare operations
  • Additional uses require valid authorization unless permitted by law
  • Authorization may be revoked in writing
  • Third parties receiving PHI are contractually restricted in use

  • Session cookies and persistent cookies may be used
  • Disabling cookies may affect functionality
